What should I wear

Wear that is comfortable to you and that will allow your therapist to evaluate what you are being treated for.

What should I bring

Fill out the new patient forms and bring them in as well as your medical insurance card. If you are referred by your physician, bring in any paper work from them as well.

What can I expect at my first appointment

On your first appointment we perform your evaluation and determine the plan of care (POC) , and visits needed.

How much will physical therapy cost

With respect to insurance, we have chosen to be out of network so that session duration is determined by individual need, instead of by an insurance company. We do not do double booking, once you schedule your session with us we do decide the length of the session based
on your need/ your condition.

We will provide you with an itemized statement, commonly referred to as a “superbill”, so that you may bill your insurance , but we do not bill insurance directly. We accept cash, checks, and credit or debit cards(including FSA and HSA cards) at the time of service. You can contact us directly for more detail on the pricing of the service you are requesting .
